(1) Vendor needs to provide temporary patient navigation services consulting services to the government authority located in Columbia, SC.
- Provides patient education, support and screening opportunities to uninsured and medically underserved individuals including medical chart review, participant interviews by phone and in-person, education visits at the client’s medical home, coordination of visits, follow up communications with clients and providers.
- Uses a virtual database to track and monitor referrals, screening and patient navigation services.
- The service categories are senior patient navigator, lead patient navigator, patient navigator level, patient navigator level, patient navigator level, junior patient navigator level and junior patient navigator level.
(2) All the questions must be submitted no later than September 27, 2018.
