[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide medical services in connection with employment matters by providing general employment physical examinations, extended sick leave evaluation, referral to employee assistance program provider and wellness program etc. for government authority located in North Charleston, SC.
(2) All questions by email must be submitted no later than October 3, 2016.
(3) Contract period will be for one year.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
