[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide health and wellness center services for acute and episodic health
care, wellness services, annual health screenings, chronic care identification and management,
outpatient occupational healthcare, and other services
- To improve employee health, welfare and workforce betterment; and
- To save City financial resources (money)
- Increase medical access for employees and insured dependents;
- Improve health outcomes for employees and insured dependents;
- Improve treatment and compliance for patients with chronic health conditions;
- Provide access to health coaching and care management;
- Improve wellness and disease management as well as wellness incentive management
and administration;
- Reduce costs for the City self-insured health plan and plan members;
- Provide health screening services;
- Reduce reliance on emergency room and urgent care;
- Improve employee productivity and reduce absenteeism;
- Provide point of integration for health plan services (care management, customer service)
with health care delivery;
- Develop occupational health services excluding worker related injuries; and
- Provide alternative options for retail pharmacy, specialty medications, and infusion services
- Potentially provide medication samples;
- Partner with the City Medical Director for EMS/EMT service needs; to the government authority located in Rock Hill, SC.
(2) All question must be submitted no later than July 23, 2018.
(3) A contract period will be for three years.

Performance of the work will be Onsite.
